Promoting Cooperation among European Regions for Capacity Building of Integrated Care Systems based on Innovative Services and Technologies

The Workshop organised by the EU project NEXES was held in Barcelona on April, 25^th 2012. We have the pleasure to invite you to join us through the videos of all the presentations available on this website.

The aim of the project has been to validate four ICT-enabled integrated care programmes in large scale trials, targeting prevalent chronic conditions (mainly Chronic Obstructive Pulmonary Disease (COPD), Congestive heart failure (CHF) and diabetes):

• Well being and rehabilitation. Promotion of early diagnosis and healthy lifestyles of clinically stable chronic patients.
• Enhanced Care. Prevention of unplanned hospitalisations in frail patients with high risk of admissions.
• Home hospitalisation & early discharge in patients with severe exacerbations of their chronic conditions.
• Support. Remote support of specialist to diagnostic and/or therapeutic procedures in Primary Care.

This Final Workshop builds on the key lessons derived from the project (which finished in May 2012), and is especially centred on the sharing of experiences and cooperation among leading European regions to foster transition towards sustainable Integrated Care services for chronic patients.

This Final Workshop was be attended by a highly qualified group of health authorities, policy makers, decision makers, researchers and other significant players in the European eHealth arena in addition to the project team and to senior representatives from the project partners.
